United Way of Northern Ozaukee is committed to delivering a more comprehensive and coordinated delivery of health and human services to our most vulnerable and at-risk population. For the second consecutive year, UWNO reinvested your dollars to support the Community Services Navigator (CSN) initiative. The CSN is a low-barrier approach to connect people quickly to the appropriate services they need to regain stability and thrive. The CSN will create a customized plan of care based on the participants' needs and clear a path to access the needed services including medical providers, mental health services, substance treatment and many other community resources. In 2021, United Way of Northern Ozaukee started funding the newly created CSN position with its Community Impact Grant. Proceeds from the fall golf outing funded the shared care coordinator resource to provide holistic care to families and individuals in our community. Advocates of Ozaukee, Lakeshore Regional Child Advocacy Center, Ozaukee Family Services and Sirona Recovery connect interagency clients to the services and resources they need to reduce their risks and improve their overall health.

CSN Mission
Collaborate with participants to identify needs and create a personalized plan on how to meet those needs. Empower participants to be independent while advocating on their behalf to increase chances of success. Follow up with participants and evaluate progress. From a strengths-based and trauma-informed approach, engage with local residents, businesses and community partners to ensure individuals and families are connected to the appropriate resources for their situation.
CSN Vision
Serve as a central guide to the resources available to Ozaukee County residents, provide an extra layer of support in hopes of reducing gaps in service, and assist participants before their situations become crises.
